Outreach Services are designed to provide support services for individuals and families within the 17 counties of Northeastern Wisconsin, which include:
- Home visits and follow up by an Community Outreach Specialist to provide support to individuals
- Resource information
- Help in obtaining financial assistance, based on need, to individuals for the purchase of assistive or adaptive equipment
- Parent and community education
A Community Outreach Specialist provides support and follow-up to help ensure that individuals and their families receive needed services. Efforts are made to work cooperatively with community resources, counties and school districts.
