Outreach Services are designed to provide support services for individuals and families within the 17 counties of Northeastern Wisconsin, which include:
- Home visits by an outreach coordinator to provide support to individuals
- Resource information
- Case management and follow-up
- Help in obtaining financial assistance, based on need, to individuals for the purchase of assistive or adaptive equipment
- Parent and community education
An Outreach Coordinator provides support, case management, and follow-up to help ensure that individuals and their families receive needed services. Efforts are made to work cooperatively with community resources, counties and school districts.
