Community Outreach/Advocacy Services

Outreach Services are designed to provide support services for individuals and families within the 17 counties of Northeastern Wisconsin, which include:

Home visits by an outreach coordinator to provide support to individuals
Resource information
Case management and follow-up
Help in obtaining financial assistance, based on need, to individuals for the purchase of assistive or adaptive equipment
Parent and community education

An Outreach Coordinator provides support, case management, and follow-up to help ensure that individuals and their families receive needed services. Efforts are made to work cooperatively with community resources, counties and school districts.
