Birth To 3 Services
As a Birth to 3 Program provider in Brown County, Cerebral Palsy, Inc. offers therapies and services to children birth to three years of age.  To determine eligibility, an evaluation is part of the child’s admission to the voluntary program.  Services are provided in natural environments (i.e., home, childcare centers, etc.) and address family needs, therapy, parent education, parent support, case management, individualized family service plan (IFSP) and transition to school. Service providers may include service coordinators, physical, occupational, and/or speech therapists.

REFERRALS: Referrals are made through the Brown County Birth to 3 Program. Referrals may be made by parents, physicians or other community resources. A county service coordinator will assist the family with questions and referral for services. To make a referral, contact the Brown County Birth to 3 Program at (920) 448-6095.

ELIGIBILITY: Eligibility is determined by either a diagnosed physical or mental condition which will likely result in a developmental delay, or an early intervention team which determines that the child is developmentally delayed. An evaluation is part of eligibility determination and includes a developmental history, parent observations, and informal clinical assessment using developmental testing measurements and observation. The developmental areas assessed are:

  • Cognitive development
  • Physical development, including vision and hearing
  • Communication development
  • Social and emotional development
  • Adaptive development which includes self help skills

SERVICE COORDINATION AND SUPPORT: Service coordination is provided to link individuals with services that enable the family and their child to achieve their objectives. Service coordinators provide support, guidance, advocacy, and case management to help families coordinate programming for their child. Follow-along insures that needed services are provided to the family while their child is enrolled in the program and after referral to other community resources.

FEES: Fees for physical, occupational, and speech therapy are billed by the agency to insurance, HMO's, Medical Assistance, and Brown County funding.
